For NDIS Providers

AI documentation built for NDIS providers.

Purpose-built templates for Support Coordinators, Allied Health, and Disability Support Workers. NDIS Practice Standards aligned. Sits alongside your PMS, never replaces it.

Built for Three NDIS Workforces

Coordination, clinical, and support roles each have distinct documentation needs. Each gets profession-tuned defaults and a tailored template library.

Support Coordinators

Levels 1, 2, and 3 — contact notes, home visits, plan reviews, monthly progress updates, incident notes, reportable incidents, handover docs. Voice-first capture for the field.

For Support Coordinators

Allied Health for NDIS

OTs, physios, speech pathologists, dietitians, exercise physiologists working with NDIS participants. Functional Capacity Assessments, Plan Reviews, Progress Reports — written the way NDIA planners expect.

For Allied Health

Disability Support Workers

Person-centred, NDIS-aligned documentation focused on support provision, participant goals, and functional outcomes. Templates built for individual support plans and behaviour support.

For Support Workers

We sit alongside your PMS, not in place of it.

Lumary, Brevity, Splose, Careview — pick the practice management system that fits your organisation. Grounded Scribe makes your documentation faster, more accurate, and more audit-defensible. We don't do PRODA claiming, NDIS Price Guide budget tracking, or rostering — that's your PMS's job. We do documentation, and we do it well.

Built for NDIS workflow

Voice-first capture, NDIS-shaped templates, care-team visibility, reportable incident support — designed around how NDIS work actually happens.

Voice-First Capture

Dictate after a home visit or care team meeting. No need to record the participant. AI generates a structured note in seconds.

NDIS-Shaped Templates

Contact notes, home visits, plan reviews, FCAs, behaviour support plans, incident notes, reportable incidents — purpose-built, not generic clinical.

Practice Standards Aligned

Built around the NDIS Practice Standards, NDIS Code of Conduct, and NDIS Quality and Safeguards Commission frameworks.

Reportable Incident Support

Dedicated NDIS QSC-aligned template that supports your portal submission within statutory timeframes.

Care Team Visibility

Multiple providers per participant, shared within your organisation with role-based access. Audit-logged by default.

Sits Alongside Your PMS

Designed to co-exist with Lumary, Brevity, Splose, Careview, and other NDIS PMSs. We do documentation; they do bookings, claims, and budgets.

Compliance the QSC will recognise

Australian hosting, encrypted at rest and in transit, automatic audio cleanup, a full audit trail, and role-based access. Built for organisations who take NDIS governance seriously.

Frequently Asked Questions

How NDIS providers use Grounded Scribe day-to-day — co-existence with PMS, reportable incidents, data hosting, and pricing.

Do you replace our practice management system?
No — and we deliberately don't try to. Grounded Scribe is the documentation and field-capture layer. Your PMS (such as Lumary, Brevity, Splose, or Careview) handles bookings, NDIS claiming via PRODA, budget tracking against the NDIS Price Guide, and provider directories. We focus on making your notes faster, more accurate, and more audit-defensible — so your team spends less time at the desk.
Which NDIS roles is this built for?
Support Coordinators (Levels 1, 2, and 3 / Specialist), Allied Health practitioners delivering NDIS-funded supports (OT, physio, speech pathology, dietetics, exercise physiology, psychology), and Disability Support Workers. Each role gets profession-tuned defaults and a tailored template library.
How does this handle reportable incidents?
We have a dedicated NDIS Reportable Incident template aligned to the NDIS Quality and Safeguards Commission framework. It supports your internal record and the underlying basis for portal submission — your team still lodges the formal notification via the NDIS Commission portal within the statutory timeframes (24 hours for most categories, 5 business days for unauthorised restrictive practices).
What about restrictive practices?
A dedicated SC Restrictive Practice Note template captures the practice used, authorisation status, triggers, duration, participant response, and less-restrictive alternatives considered. It is jurisdiction-aware — wording should be checked against the state-based authorisation framework where applicable. Unauthorised use of restrictive practices is flagged as reportable.
Where is NDIS participant data stored?
All data is stored in Australian data centres, encrypted at rest and in transit. Audio recordings are deleted immediately after transcription. Full audit logs track every access. Data is never used to train AI models.
Can a team of Support Coordinators share participant records?
Yes. Organisation accounts support shared participant records with role-based permissions and full audit trails. Handover between SCs is built in, including one-click generation of a structured handover document covering plan status, active services, outstanding actions, and key contacts.
Is there integration with NDIA / PRODA?
Not directly — PRODA bookings, claiming, and bulk upload are PMS territory, and we don't enter that lane. Grounded Scribe produces the case notes, plan-review evidence, and incident records that sit upstream of (or alongside) your PMS workflow. Export from Grounded Scribe into your PMS today is via copy-paste; deeper PMS export is on our roadmap.
Can the AI cope with NDIS-specific language?
Yes. The note-generation AI is profession-tuned. Support Coordinators receive plain-language, non-clinical, person-centred outputs; allied health professionals receive ICF-framework outputs for FCAs and progress reports; DSWs receive person-centred support documentation. Each role is configured separately and tested against NDIS-specific examples.
