Item-by-item documentation tests for Australian practitioners delivering NDIS-funded supports. Each entry covers what the support item actually pays for, what your notes and reports must demonstrate to satisfy the NDIS Quality and Safeguards Commission, and the failure modes that show up most often at plan review.
Support item codes, descriptors, and price limits are updated by the NDIA each quarter. Always verify the current line on the NDIS Pricing Arrangements and Price Limits before claiming. These pages do not include rate amounts and are not legal, clinical, or regulatory advice. Practitioners are responsible for ensuring their documentation meets their registration body, employer, the NDIS Quality and Safeguards Commission, and NDIA requirements.
