Clinical Documentation Notes

ICD-11 reclassified gender identity conditions as "Gender incongruence" under sexual health, depathologising it. DSM-5-TR uses "Gender Dysphoria." Use respectful, affirming language. Focus clinical notes on the individual's experience, goals, and any distress, rather than pathologising gender identity.
