Clinical Progress Tracking

Visualise Client Progress with Assessment Charting

Chart standardised assessment scores across timepoints, overlay multiple measures, customise colours, and export publication-ready charts — all within your client records.

How It Works

Grounded Scribe lets you add a scored assessment to a client chart, assign it to a labelled timepoint, and view trends immediately — no manual data export or spreadsheet required.

1

Administer

Complete a standardised assessment with your client — directly in Grounded Scribe or by entering scores from a paper-based administration.

2

Add a Timepoint

Each scored assessment is automatically placed as a timepoint on the client's chart. Label it Baseline, Week 4, Discharge, or anything that fits your workflow.

3

Track Progress

View trends across sessions, overlay multiple measures, customise line colours, annotate key events, and export charts for reports.

Progress You Can See at a Glance

Each chart plots scored assessment results against labelled timepoints — Baseline, mid-treatment, discharge, or custom dates — with validated severity bands (mild/moderate/severe) overlaid as reference thresholds. Reliable Change Index (RCI) calculations indicate whether score changes represent statistically reliable improvement beyond measurement error.

  • Overlay multiple assessments on one chart — compare PHQ-9 and GAD-7 side by side to track comorbid presentations
  • Colour-coded lines per assessment — fully customisable so every chart reflects your preferred visual style
  • Severity band overlays based on published cut-off thresholds provide instant clinical context within the chart
  • Add annotations at any timepoint — note treatment changes, significant life events, or clinical observations in context

Severity bands are reference information per published assessment guidelines. Clinical interpretation should be made by a qualified practitioner in the context of a comprehensive assessment.

Everything You Need for Outcome Monitoring

Grounded Scribe includes multi-assessment overlay, colour-coded series, per-timepoint annotations, severity band overlays, and one-click PDF export — all purpose-built for clinical outcome monitoring without any external graphing software.

Multi-Assessment Overlay

Plot multiple assessments on a single chart to see how different measures change in relation to each other across the course of treatment.

Flexible Timepoints

Add labelled timepoints — Baseline, mid-treatment, discharge, or custom session dates — and attach scores directly to each one.

Colour Customisation

Choose any colour for each assessment line or series. Distinguish overlapping measures at a glance with a palette that suits your workflow.

Severity Band Overlays

Visual threshold bands based on validated cut-off scores — mild, moderate, severe — provide instant clinical context within the chart.

Timepoint Annotations

Add clinical notes directly to chart timepoints — record treatment changes, significant events, or observations in context alongside the scores.

Export for Reports

Download charts as high-resolution images or include them in PDF client reports for referrals, discharge summaries, and clinical reviews.

Frequently Asked Questions

Answers to common questions about supported instruments, automatic scoring, RCI calculations, and PDF report generation in Grounded Scribe.

Which assessments can I chart?
Any psychometric instrument you use in practice can be charted. 30+ assessments have built-in digital templates (PHQ-9, GAD-7, DASS-21, PCL-5, K10, CORE-10, and more), and you can manually enter scores for any other measure — from OQ-45 and FIM to profession-specific tools like COPM, GFTA-3, or ODI.
How do I add a new timepoint?
Timepoints are added each time you record a scored assessment for a client. You can label them freely — Baseline, Week 4, Discharge, 6-Month Follow-Up — or leave them as session dates. Timepoints can be reordered and annotated after the fact.
Can I chart multiple assessments on the same graph?
Yes. When viewing a client's assessment chart, select which measures to overlay. Each assessment gets its own colour-coded line so you can compare PHQ-9 and GAD-7 side by side, or track any combination of measures that's clinically relevant.
What do the severity bands represent?
Severity bands are horizontal shaded regions based on the validated cut-off thresholds published for each assessment (e.g. mild, moderate, severe for PHQ-9). They are provided as reference information per published assessment guidelines. Clinical interpretation should always be made by a qualified practitioner in the context of a comprehensive assessment.
Can I export charts for reports or referrals?
Yes. Charts can be downloaded as high-resolution PNG images or embedded directly into PDF client reports. This is useful for including progress data in referral letters, insurance reports, NDIS progress reports, and discharge summaries.
Is assessment charting included in my plan?
Yes. Assessment charting is available at no cost on every AI Scribe plan — no add-on required.
