See Before You Send: Free Sample PDF Reports for Every Assessment on Grounded Scribe
Grounded Scribe Team
19 Apr 2026
Summary
Every assessment tool on `/tools` now has a free, downloadable sample PDF. You can preview it inline on the tool page, click to enlarge in a lightbox, or download the full PDF and keep it on file.
- 33 sample reports covering anxiety, depression, ADHD, autism, trauma, personality, cognitive screening, substance use, wellbeing, and child & adolescent instruments
- Generated with production scoring — the PDF you download uses identical scoring logic to the one a real client receives
- Profession-matched samples are automatically attached to the welcome email when a practitioner signs up, and to every assessment invitation email sent to a client
- Watermarked with a diagonal SAMPLE label so there is no confusion about clinical use
One link, zero friction — and the best way to see exactly what your clients and referrers will receive.
Why Sample Reports Matter
Assessment tools are the part of a clinical platform that practitioners hand over to other people most often. Clients complete them. Referrers read the results. Supervisors review the scoring. Each of those audiences has a reasonable first question: *what am I actually going to receive?*
Until now, the answer required trust: "It is a good-looking PDF, I promise." Now the answer is: *here is the exact PDF, take a look.*
Three audiences, three reasons samples matter:
- Clients — a concrete preview supports genuine informed consent. Completing an assessment is not an abstract ask when they have already seen what is generated at the end.
- Referrers — GPs, paediatricians, and other allied health colleagues can judge at a glance whether the report format will be useful for their decision-making.
- Supervisors and peers — a downloadable sample is what you share in a meeting to answer "how does Grounded Scribe present results?" without screen-sharing.
Coverage: 33 Tools, 33 Samples
Sample reports are available for every tool on `/tools`. A non-exhaustive list:
- Depression & mood — PHQ-9, PHQ-4, PHQ-2, CES-D-R, GDS-15, CORE-10, CORE-OM, YP-CORE
- Anxiety — GAD-7, GAD-2, PC-PTSD-5
- Trauma — LEC-5, PCL-5
- Autism & ADHD (adult) — CAT-Q, CATI, ASRS-6
- Personality — IPIP-NEO-120, IPIP-NEO-20
- Psychotic & bipolar spectrum — BPRS, CGI, AIMS
- Substance & intoxication — CIWA-Ar, URICA
- Developmental & paediatric — CDC
- Wellbeing & population screening — K-10, K-5, RSES
- Acute care — 4AT, NEWS2
- Trauma history — ACE
Each sample shows severity bands aligned with publisher guidance, subscale scores where applicable, an item-response table (or a disclosure banner if item-level display is omitted for licensing reasons), a plain-language interpretation, a score-trajectory example, licensing attribution, and the full disclaimer block.
Watermarked, So No One Confuses a Sample for a Real Report
Every sample PDF carries a diagonal SAMPLE watermark across each page. It is unmistakable. It protects practitioners from accidentally circulating a sample as a real client report, and it protects clients from assuming a demonstration PDF is about them.
Samples Are Generated With Production Scoring
The sample generator runs the same scoring engine that produces a real client report. That means when you download a sample, you are not looking at a pretty mockup — you are looking at the actual output format and scoring logic.
This matters because we recently completed a publisher-aligned scoring remediation across the library. Every sample in the current set reflects the post-remediation scoring. No legacy bands. No invented thresholds. If the publisher scoring guide sets a 0-4 / 5-9 / 10-14 / 15-19 / 20-27 structure, the sample shows exactly that.
Profession-Matched Email Attachments
Sample PDFs do not just sit on tool pages. They are built into two email flows:
Welcome Email (Practitioner Sign-Up)
When a new practitioner signs up, the welcome email automatically attaches sample reports for the assessments most relevant to their profession. A registered psychologist sees different samples than a speech pathologist, a paediatrician, or a school counsellor. Every sample is labelled and contextualised.
Assessment Invitation Email (Client)
When a practitioner sends an assessment invitation to a client, the invitation email now attaches the matching sample report. Before the client has even completed the assessment, they know what the final report will look like. That is better informed consent and a smoother client experience.
The DASS Exception
The three DASS variants — DASS-21, DASS-42, and DASS-Y — are handled differently. Samples exist, but the public `/tools/` pages for DASS are practitioner-facing only, with a "register to administer" flow. This is a deliberate decision: the authors of the DASS have explicitly ruled that public self-administration without a clinician falls outside their intended guidelines. Grounded Scribe honours that ruling.
Practitioners who register can administer DASS to their clients inside the platform and see the sample PDF there. Public calculator access has been removed.
Every other assessment on `/tools` continues to work as a standalone client-facing screener.
How to Use the Samples
- Visit `/tools`
- Open any assessment page
- Click the sample preview — it expands inline
- Download the PDF if you want a copy to share
- Use it as a reference when inviting clients, briefing referrers, or talking to colleagues about Grounded Scribe
Why This Is Practice-Building, Not Marketing
You will notice we have built the sample-report system into the product, not into a marketing landing page. That is deliberate. A sample PDF is useful during a referrer conversation, during a client informed-consent process, during a supervisor review — all moments that happen inside a practice, not inside a sales funnel.
Every tool is still free, no account needed, runs in the browser. The samples make the whole library more useful by giving every audience a concrete picture of what they are about to interact with.
